LONG BAY THEATRE
BEACH TO BROADWAY SUMMER CAMP
LIABILITY WAIVER AND RELEASE OF CLAIMS
I, the undersigned parent or legal guardian of the participant listed below, hereby give permission for my child to participate in Long Bay Theatre’s Beach to Broadway Summer Camp and all related activities, including rehearsals, classes, workshops, performances, and other camp-sponsored activities.
I understand that participation in theatre-related activities may involve certain risks, including but not limited to physical activity, dance, movement, use of props and set pieces, transportation between activity spaces, and other activities that may result in injury, illness, property loss, or damage.
In consideration for my child’s participation in Beach to Broadway Summer Camp, I voluntarily assume all risks associated with participation and agree to release, waive, discharge, and hold harmless Long Bay Theatre, its officers, directors, employees, volunteers, instructors, contractors, agents, sponsors, and representatives from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising out of or related to my child’s participation in camp activities, including claims arising from ordinary negligence.
I understand that Long Bay Theatre will take reasonable precautions to provide a safe environment; however, no activity is entirely free from risk, and participation is voluntary.
I further agree that Long Bay Theatre shall not be responsible for lost, stolen, or damaged personal property brought to camp by participants.
By signing below, I acknowledge that I have carefully read and understand this Liability Waiver and Release of Claims and agree to its terms voluntarily on behalf of myself and my child.
